Patient & Caregiver Table Invitation - Roadmap for Home and Community Care

The Ministry of Health and Long Term Care (MOHLTC) is  seeking individuals who might be interested in participating in the Patient and Caregiver Advisory Table.
A bit of background on the Patient and Caregiver Advisory Table:
In May 2015, Minister Hoskins released Patients First:  A Roadmap to Strengthen Home and Community Care (Roadmap), a three-year plan that includes ten steps to introduce greater consistency in care, a better understanding of the services available, more support for caregivers, and better access to the right care for those who need it most. The Roadmap responded to the advice received in the Bringing Care Home report from the Expert Group on Home and Community Care.

Members of the Patient and Caregiver Advisory Table will provide advice and feedback to support the effective implementation of the Roadmap, identify opportunities for meaningful engagement, and help define what success means to patients and caregivers.  It is expected that members will meet on a bi-monthly basis and when a special meeting is called.

·         A short description of why this nominee should be chosen to participate on this table, including any current or previous work representing patients or caregivers on committees or groups.
MOHLTC appreciates your support in finding voices in your community that will help them launch of the Patient and Caregiver Advisory Table, and ensure implementation of the Roadmap is truly patient- and caregiver-centred.
